Review and Follow-Up

Our team will carefully review your application. If we need more information, we will contact you directly.

Personalized Support Plan

Once approved, we'll work with you to create a plan of care. This may include housing support, financial literacy programs, workforce training, or emergency resources.

Ongoing Guidance

You won't walk this journey alone. Our case management team will check in and help guide you toward independence and stability.

APPLICATION FOR ASSISTANCE

We are a charitable organization created in part to offer general relief services and referral resources to individuals and families in need, ue to the profound needs that come to our attention. The Shepherd’s Hands Foundation of Central Ohio is unable to grant every request, but possibly a portion of the request.

Rules of Engagement:

1. Application is required (no exceptions).
2. Checks will be made payable to a business entity; no cash will be exchanged. Please provide the following information below:

3. If a food request is approved, a gift card will be provided.
4. For some, we seem to find ourselves digging a deeper hole with no way out. Recipients of funding will be expected to complete a general overview of budgeting and access to resources to help.
